It is used topically as a lotion or foam, as well as orally. Additionally, it is applied via injection with mesotherapy, usually after persistent hair loss or hair transplantation. Topical minoxidil application has received approval from the U.S. Food and Drug Administration (FDA) and the European Union for the treatment of androgenetic alopecia, which we apply in our clinic.

It should be noted that a large number of clinical studies show that off-label application in various other types of hair loss, such as telogen effluvium, alopecia, eyebrow hypotrichosis, and chemotherapy-induced alopecia, has equally good results.

Although minoxidil has a proven high safety profile, it comes with the possibility of side effects, which differ depending on the form, dose, and patient characteristics. Almost all side effects are reversible with discontinuation or adjustment of therapy.

                Side Effects by Treatment Form

The side effects observed differ depending on the treatment choice.

1. Topical Minoxidil

The topical form is the most common choice. It is administered in concentrations of 2% or 5%, with the solution often containing propylene glycol supplement, which increases minoxidil absorption but may cause irritation. The foam does not include this substance and is recommended for patients with a sensitive scalp.

Adverse effects may include:

Local irritation with symptoms of dry skin, itching, or peeling.

Seborrheic dermatitis or worsening of a pre-existing condition.

Hypertrichosis in adjacent areas, such as the forehead, temples, or face, is more frequent in women due to higher absorption of the substance compared to men.

Temporary increase in hair loss during the first 3-6 weeks of use.

The percentage of patients who develop hypertrichosis outside the treated area is estimated at 3-5%, while local irritation symptoms appear more frequently in users of the 5% propylene glycol solution.

2. Oral Minoxidil (Pills)

The oral form is used primarily in cases where topical treatment fails or causes persistent skin irritation. Numerous studies show that minoxidil pills have excellent results even at low doses (0.25-5 mg/day).

Main side effect:

Hypertrichosis can occur in areas not treated, including the face, extremities, and back. This is the most common side effect, affecting about 15% of patients according to clinical studies. The incidence is higher in women and tends to be associated with higher doses.

Rare side effects:

Quite rarely, the following side effects have been recorded in clinical practice:

Fluid retention or mild edema

Orthostatic hypotension

Tachycardia or palpitations

Dizziness, fatigue

Depression, anxiety

Adverse effects are related to dose, rate of dose increase, and age. Their occurrence is rarer at low doses, but close medical monitoring is required.

3. Injectable Minoxidil

Administration of minoxidil through micro-injections (mesotherapy) aims to enhance its local action while avoiding systemic side effects. Its safety depends on the concentration of the solution and the application technique.

Possible adverse effects that usually resolve within a few hours:

Local irritation or transient edema at the application site

Sensitivity or burning sensation after injection

More rarely, a hematoma or minor infection

The method is indicated for patients who cannot tolerate oral administration or wish enhanced local action without daily medication intake.

                Side Effects in Women

Women more frequently develop facial hypertrichosis, either due to greater systemic absorption compared to men or due to contact of the topical form with skin outside the treated area. The use of 2% topical minoxidil is considered safer for women, while the 5% concentration is associated with an increased likelihood of unwanted hair growth.

The oral form should be avoided during pregnancy and lactation, as its safety has not been fully documented due to insufficient number of clinical studies.

                Side Effects in Vulnerable Groups

Elderly

Elderly patients, especially those taking multiple medications (polypharmacy) or with a history of cardiovascular problems, should use minoxidil with particular caution and close medical monitoring.

Heart Patients

Suppose you have a history of heart failure, ischemic heart disease, or other cardiovascular problems. In that case, the use of minoxidil may be contraindicated or require extremely close monitoring and combination with other medications. Cardiac side effects appear mainly in the oral form, and usually at higher doses or in patients with predisposition.

                Managing Side Effects

Most side effects are mild and resolve with simple modifications in dosage or a change of treatment form. In scalp irritation, changing formulation (e.g., from lotion to foam) or reducing application frequency usually suffices.

It is also essential to have realistic expectations. Minoxidil requires long-term and consistent use to maintain results, as hair loss will return after drug discontinuation.

Temporary hair loss in the first weeks is expected and does not require drug discontinuation. On the contrary, continuation of treatment is recommended, as temporary loss constitutes a physiological stage of restoration of hair follicles from the telogen phase to the growth phase, where old hairs fall to make way for new, stronger ones. This is a sign that the drug is working and usually resolves within 3-6 weeks.

In case of hypertrichosis outside the treated area, dose reduction or avoiding product flow to the forehead or temples usually suffices.

More serious side effects, such as tachycardia or edema with the oral form, require discontinuation or dosage adjustment. Monitoring by a physician experienced in minoxidil use is essential for preventing complications in heart patients or vulnerable individuals.

                Helpful Tips by Treatment Type

1. If you are taking pills

Due to systemic absorption by the body, close monitoring by the doctor is required, especially for cardiovascular side effects such as changes in blood pressure, heart rate, and fluid retention.

Systemic hypertrichosis is the most common side effect. If it appears, your doctor may recommend a dose reduction or minoxidil discontinuation, which usually leads to the gradual resolution of unwanted hair. Hair removal methods, such as laser hair removal, can also be used.

Before therapy, inform your doctor about all medications and supplements you are taking, as minoxidil may interact with antihypertensives, NSAIDs, cyclosporine, and aspirin.

If you experience dizziness, taking minoxidil at night and increasing fluid intake may help.

3.
